Rectal exam revealed a palpable mass along the posterior wall located 3cm from the anal verge and colonoscopy found a 4cm friable, fungating, circumferential mass with distal aspect 3cm from dentate line and extending 4cm cranially. CT CAP demonstrates concentric rectal wall thickening with small mesolectal lymph nodes and no evidence of metastatic disease. MRI confirms a T3 tumor with shortest distance to the mesorectal fascia of 3mm and multiple round heterogeneous mesolectal/presacral lymph nodes. Multi-disciplinary review of case and patient discussion confirms plans for total neoadjuvant therapy in hopes of non-operative management.
