Case Description:
Patient is 58F with 6-month history of intermittent bright red blood in stools and change in bowel habits. Rectal exam revealed a palpable mass along the posterior wall located 3cm from the anal verge and colonoscopy found a 4cm friable, fungating, circumferential mass with distal aspect 3cm from dentate line and extending 4cm cranially. CT CAP demonstrates concentric rectal wall thickening with small mesolectal lymph nodes and no evidence of metastatic disease. MRI confirms a T3 tumor with shortest distance to the mesorectal fascia of 3mm and multiple round heterogeneous mesolectal/presacral lymph nodes. Multi-disciplinary review of case and patient discussion confirms plans for total neoadjuvant therapy in hopes of non-operative management.

Case Description:
64yo M, KPS 60, GBM (IDH WT) of the left temporal lobe with asymptomatic transient leptomeningeal (LM) enhancement, MGMT negative. 

• Presented with symptomatic hydrocephalus (acute AMS, vomiting), found to have hemorrhagic tumor and leptomeningeal enhancement of the posterior fossa and upper C-spine.
• Underwent VP shunt with improvement of hydrocephalus symptoms, and biopsy alone of the tumor.
• LM enhancement resolved on interim MRI 5 weeks later (LM enhancement felt to be secondary to tumor hemorrhage).
• After multidisciplinary evaluation and discussion, planned for hypofractionated radiotherapy with concurrent/adjuvant temodar.
• Images fused: Initial MRI (T1+C and T2; MR secondary 1 and 2) showing the extensive LM enhancement in the posterior fossa, and ~5 week follow up MRI day before simulation with resolution of LM enhancement (T1+C and T2; MR secondary 3 and 4). He was claustrophobic with some motion degradation on scans despite Ativan.

This Case will discuss EBRT and interstitial brachytherapy for a patient with locally advanced cervix cancer (LACC). The objective of this case is to identify how to contour target volumes for EBRT and brachytherapy for FIGO IIIC2 squamous cell carcinoma of the cervix. Furthermore, this case will explore the different techniques of brachytherapy in the management of LACC, the dosimetry goals for the surrounding OARs in patients requiring brachytherapy, and methods to reduce doses to these OARs.
