The patient presents with bilateral labia majora disease with involvement of the distal vagina and distal urethra. The patient has bilateral inguinal lymph nodes including ulcerating left inguinal nodes. The patient will be treated with 50Gy in 25 fractions to elective nodal and primary regions, and conedown to 64Gy total dose in 7 additional fractions with concurrent weekly cisplatin and IG-IMRT. This will include a discussion of which endometrial cancer patients require EBRT alone versus EBRT + systemic therapy versus vaginal cuff brachytherapy versus close observation after hysterectomy. The objective of this case is to identify appropriate nodal target volumes and organs at risk (OARs) for patients who require EBRT after hysterectomy for endometrial cancer.
