Workshop:
ASTRO Annual Refresher Course March 25-26 ,2026

Faculty:
Dr. Neil Taunk, MD, MSCTS

Case Description:
This is a 74 year woman with a 2cm cT1N0 multifocal primary squamous cell vaginal cancer with bulk of disease in the distal right vaginal canal, biopsy proven but intact. The patient is shown simulated full bladder (empty bladder scan not shown). She will have treatment of the entire vaginal canal given multifocal disease, whole pelvis, and bilateral groins which are clinical clinically negative by PET and MR.

==Case 01: Intact vulvar cancer with bulky inguinal nodes==

Workshop:
ASTRO Annual Refresher Course March 25-26 ,2026

Faculty:
Dr. Neil Taunk, MD, MSCTS

Case Description:
This is a 39 year old woman with cT3N3 (Group Stage IVA) Squamous cell cancer of the vulva. The patient presents with bilateral labia majora disease with involvement of the distal vagina and distal urethra. The patient has bilateral inguinal lymph nodes including ulcerating left inguinal nodes. The patient will be treated with 50Gy in 25 fractions to elective nodal and primary regions, and conedown to 64Gy total dose in 7 additional fractions with concurrent weekly cisplatin and IG-IMRT. The patient is staged with PET and MR

Non-operable vulvar cancer

Workshop:
ASTRO E-contouring GYN webinar

Faculty:
Dr. Scott Silva

Case Description:
This case will examine external beam radiation treatment in a patient with non-operable vulvar cancer. The objective of this case is to identify the primary target and nodal target volumes in patients with squamous cell carcinoma of the vulva. Of importance in the management of vulvar cancer is the correct delineation of inguinal lymph node target volumes

Homework:
Contour the tumor GTV
Contour the bilateral inguinal lymph node CTV at the level of the mid pubic symphysis
Contour the lymph node on the LEFT at the level of external iliac/inguinal junction

Locally advanced cervix cancer - Interstitial brachytherapy plan

Workshop:
ASTRO E-contouring GYN webinar

Faculty:
Dr. Scott Silva

Case Description:
This Case will discuss EBRT and interstitial brachytherapy for a patient with locally advanced cervix cancer (LACC). The objective of this case is to identify how to contour target volumes for EBRT and brachytherapy for FIGO IIIC2 squamous cell carcinoma of the cervix. Furthermore, this case will explore the different techniques of brachytherapy in the management of LACC, the dosimetry goals for the surrounding OARs in patients requiring brachytherapy, and methods to reduce doses to these OARs.

Homework:
Contour the HR-CTV
Contour the LEFT peri-rectal lymph node
Contour the more lateral LEFT external iliac lymph node at the level of S4

Post-operative endometrial cancer

Workshop:
ASTRO E-contouring GYN webinar

Faculty:
Dr. Scott Silva

Case Description:
This case explores adjuvant external beam radiation therapy (EBRT) in a woman with FIGO stage II endometrioid adenocarcinoma. This will include a discussion of which endometrial cancer patients require EBRT alone versus EBRT + systemic therapy versus vaginal cuff brachytherapy versus close observation after hysterectomy. The objective of this case is to identify appropriate nodal target volumes and organs at risk (OARs) for patients who require EBRT after hysterectomy for endometrial cancer.

Homework:
Contour the ITV_4500 at the level of S2
Contour the ITV_4500 at the most inferior extent of the external iliac lymph nodes
Contour the rectum at the level of S5
